Hospice is a compassionate, supportive service with expert medical care for patients with terminal illnesses who are facing the end stages of life.

We provide a personal touch to every patient for whom we have the honor of caring.

When curative treatment is no longer the best option for a loved one, hospice care can help to focus on quality of life.

Hospice provides pain and symptom control, and spiritual and emotional support while encouraging the highest quality life possible for patient and family alike. Dignity and comfort become the goals of care instead of curing.

CrestCare collaborates with the patient and family to bring the most compassionate levels of care while enabling the best quality and longest possible quantity of life for those in their final stages.

Who pays for hospice?

Hospice is covered 100% by Medicare Part A and Texas Medicaid. Coverage is also available through private insurance plans.

What can I expect while receiving Hospice?

Our team of expert clinicians work with the patient, family and medical director to create a comprehensive plan of care. The plan of care can include the following, but is not limited to:

  • Bereavement Counseling
  • Volunteers

Do I need Hospice?

Common Signs and Symptoms that may indicate hospice care is right for your loved one might include:

  • Multiple hospitalizations
  • Recurrent infections
  • Unintended weight loss
  • Functional or clinical decline of the patient
  • Increased assistance needed with activities of daily living
  • Shortness of breath
  • Oxygen dependence
  • Wounds that don’t heal
  • Cognitive decline

Levels of Care

Crest Hospice provides four different levels of care. The CrestCare team provides the unique approach to hospice through its seamless continuum of care through all levels of care. The experienced team through its compassionate care of patients understands these transitions can be upsetting and confusing for the patient and family. Because Crest is a locally owned and locally managed professional organization, transitions can be made quickly and are seamless between levels of care when a patient needs a different care offering.

Routine Hospice Care

This level of the Medicare Hospice Benefit is most often used in patient care nationwide. This level of care can be delivered in any place the patient calls home. Visits are scheduled by a team of hospice professionals to ensure that medical, spiritual, emotional and personal care needs are met.

Continuous Care

When a patient and family experience a crisis, MyCrestCare can arrange a level of care that helps alleviate the crisis with nursing care provided for 24 hours post crisis. Once the situation subsides, the MyCrestCare Case Manager can begin routine home care again.

General Inpatient Care

If a patient experiences pain or symptoms that cannot be managed in the patient’s home, the MyCrestCare team can assist by transferring the patient to a contracted in-patient facility. Once the situation is resolved, the team will facilitate the patient’s transfer home.

General Inpatient Respite Care

MyCrestCare understands that care giving is very hard work and can be difficult for care givers. When care givers need time away from their caregiving responsibility, patients can stay in a contracted inpatient facility for up to five days of care.

The CrestCare hospice team is available 24 hours a day 7 days a week to serve the needs of our patients.
